Choice & Control
The Consumer Directed Model of service, often referred to as Consumer-Directed Care (CDC) or Self-Directed Services, empowers individuals with disabilities, health challenges, or the elderly to have greater control over the services they receive. Individuals have the autonomy to choose their own attendants. This promotes independence and allows for personalized care.
Management of Attendant Hours
Individuals will be approved by their authorizing agent for a specific number of hours per week or a budget of hours if you will, to manage their care. This gives the Individual control of the attendant schedule to best meet their unique needs.
Empowerment & Support Systems
The model aims to empower consumers by involving them in decision-making processes about their care and support. This can improve satisfaction with services and overall quality of life.
To facilitate this model, individuals will have access to Services Facilitation by an approved Medicaid Provider, such as Toddlers To Grandparents. The individual will also utilize a Fiscal Employer Agent (PPL or CDCN) who is contracted through the Department of Medical Assistance Services.
Consumer Directed VS. Agency Directed
Consumer Directed contrasts with Agency Directed in many ways. With Agency Directed, the providers makes decisions on behalf of the consumers, which can sometimes lead to a one-size-fits-all approach that doesn't fully address individual needs or preferences. Agency Directed does not allow you to choose your own attendant, one is attached/assigned to you. Overall, the Consumer Directed Model fosters empowerment and self-determination for those receiving services.
To Utilize the Consumer Directed Model
An EOR must be established. The EOR is responsible for hiring, training, supervising and termination of the attendant who provides the direct support to the waivered individual. A Services Facilitator, like TTG, must be chosen to begin services. You must agree to routine in-home reviews of your services with your SF held on a monthly or bi-monthly basics according to your MCO schedule.
Legally Responsible Individual (LRI)
A legally responsible individual is any person who has a duty under state law to care for another person. This includes parents of children under the age of 18 (this includes biological, foster, stepparents and legal guardians) and spouses.
Meet Tiffany, Age 61
Tiffany suffered a stroke and Brain damage from an anoxic brain injury. She also has dystonia which means her body suffers from involuntary muscle contractions which leads to repetitive and twisting movements.
