Toddlers To Grandparents is looking for compassionate, organized, and dedicated Service Facilitators to join our growing team.
What’s a Service Facilitator?
A Service Facilitator helps individuals and families who receive Medicaid consumer-directed services manage their in-home care. Instead of providing hands-on care, you’ll act as a guide — making sure families understand the program, helping them hire and manage their own caregivers, keeping records up-to-date, and ensuring Medicaid rules are followed. You’ll visit families in their homes, check that care needs are being met, offer resources, and provide support when challenges arise. It’s a role that blends community connection, problem-solving, and advocacy — and every day, you’ll be making a difference.
We are hiring statewide in Virginia, with high demand in:
- Chesterfield County
- Richmond City
- Chester
- Middle Peninsula
